The health & care sector ten kilometres from France

On a ward in Tournai as in a community health centre in Mouscron or Kain, it happens that patient and team share no language at all. What goes unsaid at that moment cannot be recovered later: a symptom stays unnamed, a consent loses its force, a dosage is misunderstood. Our healthcare interpreters hold to the rule of the trade, which is to convey everything that is said, including what is uncomfortable, without softening, without summarising and without answering in the clinician's place.

The nearness of the border keeps files moving. Care nomenclatures do not overlap from one country to the next. A procedure translated word for word can point, on the Belgian side, to something that opens no entitlement at all, and the reimbursement collapses. We keep the original heading, add the counterpart where one exists and say so where none does: an institution prefers an explicit note to a convenient equivalence.

We do, in both directions. Reports cross the border every week and sometimes go back completed by a Belgian specialist. We keep the units and reference values the final reader expects, and note in the margin anything with no counterpart in their system.

Which languages do care services here ask for most?

Dutch and the French of France, because of the border, then Arabic, Turkish, Romanian and Polish. English mostly concerns patients passing through. All of them sit within the more than 70 languages we handle, and a sworn interpreter is booked within 24 h.

What do you do when a patient turns up unannounced?

We open the phone line, usually within the hour, while an interpreter sets off if the stay is going to last. Our working day runs from 8am to 10pm, Saturday and Sunday included, because unplanned arrivals do not keep office hours.

Who proofreads a consent form?

A second linguist from the field, after the translator. The text has to stay readable for someone with no medical training while sticking to the protocol, and those two demands often pull against each other. That trade-off is what justifies the double reading, far more than terminology.
